Cholinerge signaaloverdracht
Cholinerge signalering is een fundamenteel neurotransmittersysteem dat zowel in het centrale als in het perifere zenuwstelsel werkt en snelle transmissie medieert bij de neuromusculaire junctie, autonome ganglia en in de hersenen, waar het aandacht, leren, geheugen en arousal reguleert.
Acetylcholine (ACh) wordt gesynthetiseerd uit choline en acetyl-CoA door choline-acetyltransferase (ChAT) en verpakt in vesikels door VAChT (Oda, 1999; Parsons et al., 1993). De signalering wordt beëindigd door acetylcholinesterase (AChE), die ACh binnen milliseconden hydrolyseert; de vrijgekomen choline wordt gerecycled via de hoog-affiniteits cholinetransporter (CHT1) (Soreq & Seidman, 2001). ACh werkt via twee receptorclasses: ionotrope nicotine-receptoren (nAChRs), die pentamere kationkanalen zijn, en metabotrope muscarine-receptoren (mAChRs), die G-eiwit-gekoppeld zijn (Changeux, 2010; Wess et al., 2007). Spier-type nAChRs mediëren neuromusculaire contractie, terwijl neuronale nAChRs (bijv. α4β2, α7) snelle transmissie en presynaptische modulatie in de hersenen mediëren (Dani & Bertrand, 2007).
De vijf muscarine-subtypen (M1-M5) koppelen aan Gαq (M1, M3, M5) of Gαi/o (M2, M4); M1-receptoren zijn cruciaal voor cognitie, M2 zijn presynaptische autoreceptoren en M3 mediëren perifere parasympathische responsen (Caulfield & Birdsall, 1998). Centrale cholinerge neuronen ontspringen in de basale voorhersenen (inervatie van cortex en hippocampus) en het pedunculopontine tegmentum (regulatie van arousal), terwijl perifere cholinerge signalering alle parasympathische postganglionaire en somatomotorische functies controleert (Mesulam, 2004; Picciotto et al., 2012). Cholinerge dysfunctie ligt ten grondslag aan de ziekte van Alzheimer, waarbij degeneratie van de basale voorhersenen cognitieve achteruitgang veroorzaakt, symptomatisch behandeld met cholinesteraseremmers (Francis et al., 1999). Myasthenia gravis ontstaat door een auto-immuunaanval op spier-nAChRs, behandeld met pyridostigmine (Drachman, 1994). Organofosfaatvergiftiging remt irreversibel AChE, behandeld met atropine en pralidoxime (Eddleston et al., 2008). Nicotineverslaving wordt aangedreven door nAChRs in het ventrale tegmentale gebied (Picciotto et al., 1998). Subtype-selectieve muscarine-modulatoren worden onderzocht voor Alzheimer en schizofrenie (Conn et al., 2009).
Concluderend controleert cholinerge signalering, via snelle nicotine- en langzamere muscarine-acties, neuromusculaire, autonome en cognitieve functies, waarbij dysfunctie centraal staat bij Alzheimer, myasthenia en verslaving.
