GABA-erge signaaloverdracht
GABAerge signalering vormt het belangrijkste inhibitoire neurotransmittersysteem in het rijpe zoogdierlijke centrale zenuwstelsel en levert de essentiële tegenwicht aan glutamaterge excitatie, waardoor neurale netwerkdynamiek, ritmische oscillaties en informatieverwerking worden gevormd. γ-Aminoboterzuur (GABA) wordt uitsluitend gesynthetiseerd uit glutamaat door glutaminezuurdecarboxylase (GAD), dat in twee isovormen bestaat, GAD65 en GAD67, waarbij GAD67 de basale GABA-niveaus handhaaft en GAD65 de gemakkelijk vrijgeefbare pool voor synaptische transmissie levert (Erlander et al., 1991; Soghomonian & Martin, 1998). Hoewel ze slechts 20-30 % van alle centrale synapsen vertegenwoordigen, oefenen GABAerge interneuronen een onevenredige controle uit over de neuronale excitabiliteit dankzij hun strategische perisomatiische en dendritische targeting van principale cellen (Somogyi et al., 1998). GABA wordt verpakt in synaptische vesikels door de vesiculaire inhibitoire aminozuurtransporter (VIAAT) en vrijgegeven door calciumafhankelijke exocytose; de signalering wordt beëindigd door hoogaffiniteits natriumafhankelijke GABA-transporters (GAT-1 op neuronen, GAT-3 op astrocyten), die GABA uit de spleet verwijderen en recycling via de glutamaat-glutaminecyclus mogelijk maken (Borden, 1996; Bak et al., 2006).
GABA werkt via drie receptorKlassen: ionotrope GABAA- en GABAC-receptoren, die snelle chlorideafhankelijke inhibitie mediëren, en metabotrope GABAB-receptoren, die langzamere G-eiwitgekoppelde responsen mediëren (Bormann, 2000; Bettler et al., 2004). GABAA-receptoren zijn pentamere assemblages, typisch samengesteld uit twee α-, twee β- en één γ-subeenheid, met 19 bekende subeenheden die een enorme functionele diversiteit bieden; het meest voorkomende subtype is α1β2γ2, dat de klassieke benzodiazepinegevoelige inhibitie medieert (Olsen & Sieghart, 2008). Bij binding van GABA opent de receptor een chloridepermeabele porie; de richting van de chlorideflux hangt af van de chloridegradiënt, die wordt bepaald door de kalium-chloride-cotransporter KCC2. In onrijpe neuronen maakt verhoogd intracellulair chloride GABA depolariserend, een ontwikkelingsomschakeling die essentieel is voor vroege netwerkvorming (Rivera et al., 1999; Ben-Ari, 2002). GABAB-receptoren zijn obligate heterodimeren (GABAB1/GABAB2) die koppelen aan Gαi/o, adenylaatcyclase remmen, calciumkanalen moduleren en GIRK-kaliumkanalen activeren, wat een langzame hyperpolarisatie produceert (Luscher et al., 1997). Een kenmerkend kenmerk van GABAerge signalering is de uitgebreide heterogeniteit van interneuronen, geclassificeerd op morfologie en markers zoals parvalbumine (PV+), somatostatine (SST+) en vasoactief intestinaal peptide (VIP+); PV+-mandcellen leveren krachtige perisomatiische inhibitie die de spike-output controleert, terwijl SST+- en VIP+-interneuronen respectievelijk dendritische integratie en disinhibitie moduleren (Ascoli et al., 2008; Tremblay et al., 2016). Naast snelle synaptische («fasische») inhibitie mediëren extrasynaptische GABAA-receptoren die δ- of α4/6-subeenheden bevatten een aanhoudende «tonische» inhibitie in respons op ambient GABA, waardoor de globale neuronale excitabiliteit wordt vastgesteld (Farrant & Nusser, 2005; Belelli et al., 2009).
GABAerge signalering is het primaire doelwit voor verschillende belangrijke geneesmiddelenklassen. Benzodiazepines binden aan de α-γ-subeenheidinterface en versterken allosterisch de frequentie van kanaalopening om anxiolytische, sedatieve en anticonvulsieve effecten te produceren (Rudolph & Knoflach, 2011). Barbituraten verlengen de duur van de kanaalopening, terwijl algemene anesthetica zoals propofol ook de GABAA-functie versterken; flumazenil fungeert als antagonist van de benzodiazepine-bindingsplaats (Möhler et al., 2002). De GABAB-agonist baclofen wordt gebruikt bij spasticiteit (Bowery, 2006). Dysfunctionele GABAerge inhibitie ligt ten grondslag aan talrijke neurologische en psychiatrische aandoeningen. Verminderde inhibitie predisponeert tot epilepsie, en mutaties in GABAA-receptorgenen (bijv. GABRG2) zijn geassocieerd met genetische epilepsiesyndromen (Macdonald et al., 2010). Verstoorde GABAerge functie, met name in PV+- en SST+-interneuronen, is geïmpliceerd bij schizofrenie, terwijl een onbalans tussen excitatie en inhibitie bijdraagt aan autismespectrumstoornissen (Lewis et al., 2012; Rubenstein & Merzenich, 2003). Bij de ziekte van Alzheimer draagt verstoring van GABAerge circuits bij tot netwerkhyperexcitabiliteit (Villette & Villette, 2015). Therapeutische strategieën omvatten subeenheidselectieve positieve allosterische modulatoren met verbeterde bijwerkingsprofielen en neuroactieve steroïden die extrasynaptische receptoren targeten, zoals geïllustreerd door brexanolon voor postpartumdepressie (Kanes et al., 2017). Samenvattend werkt GABAerge signalering via diverse receptorsubtypen, interneuronklassen en fasische versus tonische modi om essentiële inhibitie te bieden, en haar disfunctie is centraal in een breed spectrum van neuropsychiatrische ziekten, waardoor het een kritisch doelwit is voor therapeutische interventie.
